- What do you need?
A Greensleeve on your Refrigerator – A medical Greensleeve is a plastic folder that holds important documents regarding your health care wishes. It may contain your Advance Care Plan, MOST form (medical orders for scope of treatment, medication list, emergency call numbers, allergy list, Representation Agreement. This is what paramedics look for on your refrigerator in order to follow your health care wishes. - A Will
- A Health Representation Agreement

What might you need?
- Mobility aids such as a cane or walker to ensure you don’t fall
- Support with personal hygiene, medications, etc. – Fraser Health Home Health Care
- New clients can contact our Home Health Service Line at 1-855-412-2121 (open seven days a week, 8:30 a.m. – 9:00 p.m.; on statutory holidays between 8:30 a.m. – 4:30 p.m.).

Assisted Living
Where are you living?
In a private pay Assisted Living building (you pay the full amount each month) OR in a Fraser Health subsidized Assisted Living Building (your monthly rate of pay is subsidized by Fraser Health).
Cost: 70% of net income from last year’s income tax assessment
Private Pay Assisted Living is available in Abbotsford. $6000+ per month.
Terrace East Assisted Living has both options available.
What is life like for you?
- You are assessed and QUALIFY for help with personal services
- You need help with meal preparation, laundry and housekeeping
- You may need assistance with personal hygiene, dressing and medications
- Arrange shopping and banking for yourself – you make your own decisions
- Walk with relative ease (may be using a walker or cane)
- You appreciate and may use the 24-hour response system (call bell)
What do you need?
An assessment to qualify for Assisted Living (Fraser Health: 1.855.412.2121)

Long-Term Care
Where are you living?
In a long-term care home that has 24/7 nursing care and a locked environment. The buildings are coded and locked for the safety of the residents. Residents who are safe to themselves and others, but need additional nursing care are able to come and go on their own from the long-term care home.
Fraser Health subsidized cost: 80% of net income from last year’s income tax assessment
Private Pay long-term care is available at a more significant cost – it can be $6000 – $10,000+ per month in Abbotsford (2024)
Menno Home and Menno Hospital are long-term care homes.
What is life like for you?
- You are assessed as needing 24/7 nursing care for either mobility or cognitive needs
- You need a caregiver to assist you with your daily activities of living (ADSL’s)
- You receive and enjoy all meals, laundry, recreation, personal hygiene, dressing and medication support
- Your Primary Contact is chosen, usually the Power of Attorney or spouse to communicate with the nursing team
- You have scheduled and unscheduled care provided for you
- You appreciate and may use the 24-hour response system (call bell)
